Incident Post-Mortem: Intermittent Service Outage – July 30, 2025
Summary:
On the morning of July 30, 2025, some users of the PEAK 15 Systems app experienced errors when attempting to load certain iframe-based components. This issue began around 1:30 AM PST and was fully resolved by 8:30 AM PST.
What Happened:
At approximately 6:30 AM PST, our Support team began receiving reports of error messages appearing in various parts of the application. These were primarily tied to specific iframe-based content not loading as expected.
The issue was immediately escalated to our engineering team. During our investigation, we identified that a routine overnight system maintenance task (a restart of key services) had not completed successfully for a portion of our infrastructure. This caused some—but not all—services to remain unavailable.
At 8:00 AM PST, our engineering team performed an additional system restart to bring all services back online. By 8:30 AM PST, normal functionality was confirmed to be fully restored across the platform.
